From the earliest days of my work as a therapist, I have had a continuing interest in how best to increase “the watts of influence” and cost-effectiveness of what takes place in the encounter between any change agent and a client – whether the change agent is a psychotherapist, physician, social services worker, or probation officer. A second issue is what role, if any, do stories, metaphors, analogies, confrontation, soothing, education, and coaching play in therapy? Third, what can change agents learn from the communication style of religious leaders, politicians, and from each other, and, fourth, how does the art of powerful communication become a set of definable skills that can be taught so that the change agent retains them over time?

I believe that the Motivational Interviewing style holds promise for those client-encounters in which there is ambivalence to change on the part of certain client populations such as: smokers, drinkers, the medically non-compliant, depressed/anxious trauma victims, the passive-dependent, and those ordered into therapy by judges, probation officers, and unhappy spouses with divorce lawyers on speed dial.

Simultaneously, it often seems that we want, and we don’t want, our butter, cigarettes, welfare dependency, and desserts. Controlled weight, sobriety, improved breathing, and the pride that accompanies getting and keeping a job are off in the poorly-defined future somewhere. The status quo is seductive.

Motivational Interviewing is an evidence-based, exciting, and respectful approach to treatment that nicely accommodates the ambivalence that many patients/clients feel about the changes they entertain such as smoking cessation, medical compliance, returning to work, or assuming the responsibilities of parenthood and self-reliance.
